Post-Acute Care Physician:

Candidate Requirements:

1. Halifax Health Employed Physician: Internal Medicine,
2. Family Medicine – Generally the same as a Hospitalist.
3. A Board-Certified MD or DO with a Florida License Experience

Program Overview:

     The person in this position will be under contractual agreement (Agreement) as an employee (Employee) of Halifax Staffing, Inc. (Company), and will provide professional services as medical director of post-acute care facilities - defined to include, but not mited to: skilled nursing facilities, long term acute care hospitals, nursing homes, assisted living facilities (PAC Facilities), with which the Halifax Hospital Medical Center special taxing district (District) has entered into an agreement to provide medical director services. In addition, the Employee shall provide attending physician services for patients of a PAC Facility with which the District has entered into an agreement.

     The Employee's duties shall also include: providing Medical Director oversight for non-physician employees of the Company who are providing services. The employee will keep and maintaining appropriate records relating to all professional services rendered by Employee under the Agreement; preparing in connection with such services all reports, claims and correspondence necessary and  appropriate in the circumstances; taking reasonable steps to maintain and improve his/her professional skills; and assisting the Company and the District in recruiting new physicians. All medical care rendered by the Employee shall at all times meet the standards of performance called for under the Agreement and by the District.

     The Employee’s duties shall include providing attending physician services for Halifax Health patients discharged to PAC Facilities who may not have a current agreement with the District.
