This team tries to ensure that people remain in the community, rather than them moving onto residential care. You will be involved with support planning, and potentially moving cases onto the community review team.
If you are successful you will have the potential to progress in a range of ways.

The Team manager offers full support, as they progressed to that role from a social worker, they fully understand the role and the work involved.

2 years of experience needed, within a similar team & HCPC registration. Experience within a team that supports people with disabilities beneficial.

You can expect:

* Shared work load, the team works very closely and all rely on each other to ensure that the workload is shared

Salary: £30,998.00-£37,12.00 dependant on experience
Location: North west London
