Lead Social Worker - Low Secure Mental Health Hospital - Stafford

Our client based in Stafford is currently looking for experienced Qualified Social Workers to join their Children's Mental Health Hospital. They offer children and young people with mental health needs a tranquil environment where they can recuperate and recover.

Requirements:

  • Extensive experience within Mental Health
  • Qualified Social Worker
  • HCPC Registered

Main responsibilities:

  • Manage the service provision for a defined caseload of patients / service users.
  • To act as care co-ordinator in line with THG standards
  • To ensure appropriate risk assessments are completed
  • To respond to requests for assessments and review of need working in partnership with user, carers and other professionals as agreed within the multi-disciplinary team and Lead Social Worker
  • To develop and deliver evidence based packages of care including specific psychosocial interventions where appropriate
  • To delegate the delivery of specific interventions to support workers / social work assistant as appropriate
  • To work in partnership with service users, carers, other team members and other statutory and non-statutory agencies
  • To liaise closely with other agencies at the point of discharge to ensure a smooth transition
  • To participate in caseload supervision and workload management on a regular basis as required by the team manager
  • To undertake carers assessment and care planning
  • To maintain contemporaneous health and social care records and comply with relevant THG polices
  • To ensure that all relevant data is entered onto THG information systems
  • To positively address issues of discrimination and social exclusion
  • To lead the provision of therapeutic group work as indicated by the needs of the service users
  • To use the results of and participate in clinical audit and research as required
  • To act as mentor, supervisor and assessor to students who are placed within the team.
  • To participate in supervision, both personally and as a supervisor for junior staff within the requirements of THG policy
  • Undertaking other duties appropriate to the post as requested by Lead Social Worker
  • Provision of case reports when required (e.g. for courts, mental health review tribunals / managers hearings, case reviews, enquiries etc) and attending case discussions, conferences and reviews as required including those integral to adult and child protection procedures
