You will be involved in support planning and if needed progressing cases onto a community review team.

Responsibilities for the role:
- Carry out Needs- Led assessments of service users and their carers
- Facilitate and foster effective relationship between local authority, NHS and private sector partners
- Update care plans following reviews, and negotiate care packages with service providers
- Provide care plans to service users
The vacancy opened up due to the Local authority taking measures to reduce locum social workers to encourage stability to the service. As a result of this project, the local authority is really looking to invest in their social workers and promote a "grow your own" policy. This means more chances for promotion and long term gain.
